We help place or relocate Alzheimer's and dementia patients, individuals on probation and parole, and behavior clients (mental illness and multiple diagnosed). LEAVE IT TO USThe Behavior Referral Network was established to assist county officials, probation officers, health and welfare managers, discharge planners, case managers, and others looking to find appropriate placement and care for individuals with behavior issues. Take advantage of our connections and experience in matching the right facility to the individual. Our staff includes those from medical centers, Huntingtons and TBI units, those with Vas background in MDS, psychologists, psychosocial rehab specialists, and those with mental health clinic admissions and discharge planning experience. That depth of experience is critical to finding the ideal facility to meet the unique and specialized needs of those with behavior issues, as the key to a successful placement is to match the behavioral problem with a facility that is designed and staffed to care for such individuals. THE REFERRAL PROCESSComplete the ONLINE FORM, including client name, social security number, DOB, previous admissions to any facility or hospital in the last 5 years, financial party, and Dx—that will get the process started. We may ask for additional information, such as a family history, social history, H&P, DPOA, POST, Living Will, and any doctor’s and nurse’s notes. If the preliminary data is already compiled on a face sheet or other record, then you can simply fax it to us to begin the referral process.
